Digital prescriptions in India — what is legally required?
A digital prescription in India must carry the same information as a paper one: the practitioner's name, qualification and registration number, the patient's name and age, the date, the medicine with strength, dosage and duration, and the practitioner's signature. Telemedicine prescriptions additionally follow the Telemedicine Practice Guidelines 2020.

The confusion around digital prescriptions in India usually comes from an assumption that a digital prescription is a different legal object from a paper one. It is not. It is the same prescription, recorded differently, and it must contain the same things.
What every prescription has to contain
- Who wrote it
- The practitioner's name, qualification and registration number with the relevant council. This is the part most often missing from prescriptions generated by generic software, because the software was never built for Indian practice.
- Who it is for
- The patient's name and age. Weight matters where dosing depends on it, particularly for children.
- When
- The date it was issued. A prescription without a date cannot be dispensed reliably.
- What
- The medicine, its strength, the dosage, the route where relevant, the frequency and the duration. Generic names are preferred and, for practitioners under the National Medical Commission's regulations, are expected.
- Signed
- The practitioner's signature. On a digital prescription this may be a digital or electronic signature. What matters is that it is attributable to the practitioner and not editable afterwards by someone else.
Who may prescribe what
This is where practices most often get it wrong, and it has nothing to do with the technology. Prescribing rights depend on your registration and qualification. A physiotherapist's scope in India does not include prescribing scheduled medicines, and no software changes that.
What a physiotherapy practice does issue is a treatment prescription: the exercise programme, load and progression, activity modification, and advice. That is a clinical instruction to the patient, and it benefits from exactly the same discipline — dated, attributable, specific, and given to the patient in a form they can refer back to.
Telemedicine consultations
Consultations conducted remotely fall under the Telemedicine Practice Guidelines issued in 2020. In outline, these require the practitioner to:
- Verify the patient's identity and their own, and record that they did.
- Obtain and record the patient's consent to a remote consultation.
- Maintain a record of the consultation as they would for an in-person one.
- Prescribe within the categories of medicine the guidelines permit for the mode of consultation used.
- Issue the prescription to the patient in a retrievable form.
Record keeping
- Keep a copy. A prescription you cannot produce later is a problem in exactly the circumstances where you most need it.
- Make it tamper-evident. The advantage of a digital prescription over a photograph of a paper one is that it can be attributable and unaltered. If your system does not preserve that, you have lost the main benefit.
- Keep it with the case record. A prescription that lives in a messaging app is not part of the clinical record.
- Handle the data properly. A prescription is personal health data. Under the Digital Personal Data Protection Act 2023 it should be collected for a stated purpose, kept secure, and not used for anything else.
Practical points that save trouble
