The market
How physiotherapy clinics in India actually operate.
A physiotherapy practice bills by session, not by visit, and a patient is on a course of 8 to 12 sessions rather than a one-off appointment. Almost every generic clinic system in India models the appointment and not the course, which is why physio front desks end up tracking sessions-remaining in a paper register alongside the software they paid for.

Course planning
The physiotherapist commits to a number of sessions and a frequency. This is the commercial unit. Most clinics sell it as a package and collect part of it upfront.
Session delivery
Each session logs what was done, what changed, and the home exercise assigned. Sessions are often delivered by a different therapist from the assessor.
Home programme
Exercises are handed over as video or print. Adherence between sessions is the single largest driver of outcome and the thing nobody tracks.
Reassessment and discharge
Repeat the assessment measures, compare against baseline, and discharge with a written summary. Clinics that skip the written discharge lose the referral.

- A physiotherapy enquiry is condition-shaped — sciatica, frozen shoulder, post-operative knee. Grounding the agent in the pages the practice actually wrote means it describes the approach that clinic takes rather than a general account of the condition from somewhere else.
- The course model, so the agent quotes a course and not a visit
- Physiotherapy is sold in courses of eight to twelve sessions. An agent that answers the price question with a single-session figure understates the commitment, and the correction lands at the front desk on day one.
- Live diary availability across clinics and home visits
- The valuable outcome is a booking, and a booking needs a real slot at the location nearest the patient. An agent reading a stale timetable books appointments that do not exist, which is worse than a contact form.
- A defined handover to a physiotherapist
- Anything that becomes clinical — what is causing this pain, should I be exercising — has to route to a qualified person with the thread attached, so the patient does not repeat themselves and the clinician starts informed.

What changes the build.
- Physiotherapists are not prescribers
- A physiotherapy record is a case sheet and treatment plan, not a prescription. Software that presents a prescription pad puts the practitioner in a regulatory position they do not hold. The document model has to reflect that.
- Multi-therapist attribution
- Sessions in a single course are delivered by different therapists. Both payroll and clinical accountability need per-session therapist attribution, not per-patient.
- DPDP Act consent for patient data
- Patient records and exercise videos are personal data. Consent capture, purpose limitation and a deletion path need to exist in the data model from the start, not be retrofitted as a privacy page.

One engagement, in three parts.
- Context
- A three-clinic physiotherapy practice in south Bengaluru, also running home visits and online consultations, was fielding the same questions by phone all day and losing the enquiries that arrived after the front desk went home. The questions were consistent: what does this condition involve, how many sessions will I need, what does it cost, which clinic is nearest to me.
- What was built
- An assistant grounded in the practice’s own condition pages, so it answers about the practice’s approach rather than about the condition in general. It states assessment and per-session fees, explains the typical course length, identifies the nearest of the three clinics, and books the appointment at the end of the conversation instead of collecting a number for somebody to call back. Anything clinical hands over to a physiotherapist with the thread attached.
- What changed
- The enquiries that used to be lost overnight now arrive as bookings — the assistant handles the eleven-at-night ones, which were previously a voicemail at best. Daytime calls changed shape too: patients arrive already knowing the likely course length and the fee, so the conversation starts at scheduling rather than at price.
What goes wrong
The mistakes we see in physiotherapy clinics.
- Ending the conversation with a callback request. The patient who was ready to book at eleven at night is not ready at ten the next morning, and that gap is most of what the agent was bought to close.
- Letting the agent answer clinical questions because it can produce a plausible answer. Assessing a specific person’s pain is clinical work, and a confident wrong answer in physiotherapy sends somebody to exercise on an injury.
- Quoting a per-session fee without the course context. It reads as cheaper than the commitment actually is, and the practice spends the first appointment resetting expectations it created itself.
- Grounding it in general physiotherapy content off the internet. The practice’s protocol, session length and approach are the things that differentiate it, and a generic agent describes a competitor just as accurately.

Yes, and it should — that is the difference between an agent and a contact form. It needs live diary availability and the course model behind it, so the slot it offers is real and the commitment it describes is the actual one. Ending in a callback request loses the after-hours enquiries that justify the build.
What should a physiotherapy chatbot refuse to do?
Assess a specific person’s condition, suggest exercises, or estimate recovery for an individual. It can explain what a condition generally involves and how the practice approaches it, then hand to a physiotherapist. We test those refusals adversarially before launch rather than trusting the instruction.
