A physiotherapy practice bills by session, not by visit, and a patient is on a course of 8 to 12 sessions rather than a one-off appointment. Almost every generic clinic system in India models the appointment and not the course, which is why physio front desks end up tracking sessions-remaining in a paper register alongside the software they paid for.

  1. Assessment

    First visit is 45 to 60 minutes and produces a case sheet, not a prescription — range-of-motion measurements, pain scale, and a provisional course length.

  2. Course planning

    The physiotherapist commits to a number of sessions and a frequency. This is the commercial unit. Most clinics sell it as a package and collect part of it upfront.

  3. Session delivery

    Each session logs what was done, what changed, and the home exercise assigned. Sessions are often delivered by a different therapist from the assessor.

  4. Home programme

    Exercises are handed over as video or print. Adherence between sessions is the single largest driver of outcome and the thing nobody tracks.

  5. Reassessment and discharge

    Repeat the assessment measures, compare against baseline, and discharge with a written summary. Clinics that skip the written discharge lose the referral.

Razorpay or PhonePe payment links
Package payments are usually part-paid. The system has to hold an outstanding balance against a course and settle it across several future visits, which a one-shot payment gateway integration does not handle.
Video hosting inside the patient record
Home programmes are video. Hosting them against the patient rather than as unlisted YouTube links keeps them playable on low bandwidth, lets the therapist change the programme without resending anything, and stops the clinic giving away the exercise library it charges for the moment one patient forwards a link.
Tally or Zoho Books
A course sold as a package and consumed across two months is revenue recognised over time, and most Indian clinics hand their accountant a session register to reconcile by hand. Pushing the settled portion rather than the collected amount is the difference between books that close in a day and books that close in a week.
Biometric or PIN attendance for therapists
Therapist pay is often a share of sessions delivered rather than a flat salary, so per-session attribution is a payroll input, not an audit nicety. Tying attendance to the session log removes the monthly argument about who did what.

Physiotherapists are not prescribers
A physiotherapy record is a case sheet and treatment plan, not a prescription. Software that presents a prescription pad puts the practitioner in a regulatory position they do not hold. The document model has to reflect that.
Multi-therapist attribution
Sessions in a single course are delivered by different therapists. Both payroll and clinical accountability need per-session therapist attribution, not per-patient.
DPDP Act consent for patient data
Patient records and exercise videos are personal data. Consent capture, purpose limitation and a deletion path need to exist in the data model from the start, not be retrofitted as a privacy page.

Context

What was built
A course-based patient management and billing system: one case sheet per patient, sessions drawn down against a paid package, per-session therapist attribution, and reminders that name the session number.
What changed
Package balance reconciliation moved out of the spreadsheet entirely. The month totals itself as sessions are logged, which removed the end-of-month reconciliation pass.

What goes wrong

The mistakes we see in physiotherapy clinics.

Written down because they are the part of this page hardest to fake, and the part most worth reading before you commission anything.
  • Modelling the appointment instead of the course. Every downstream problem — balances, no-shows, therapist payroll — traces back to this one decision, and it is the expensive one to change later because it is the shape of the database.
  • Building a prescription screen because the generic clinic template had one. A physiotherapy record is a case sheet and a treatment plan; a prescription pad puts the practitioner in a regulatory position they do not hold.
  • Attributing a course to one therapist. Sessions inside it get delivered by whoever is on shift, and both payroll and clinical accountability need it recorded per session.
  • Treating the exercise library as content rather than as data. If a video is not attached to the plan it was prescribed in, nobody can tell later what the patient was actually asked to do.

What physiotherapy clinics ask.

Can the software handle a patient who pays for 10 sessions upfront?

Yes, and this is the case most generic clinic systems get wrong. A package payment creates a credit that draws down as sessions are logged, so the front desk sees sessions remaining and outstanding balance without a separate register.

Should we build or buy physiotherapy practice software?

Buy, unless the practice does something a finished product genuinely cannot model. Most clinics need patient records, case sheets, a course balance and reminders, which is a solved problem at a published price. Commissioning makes sense at multiple locations, unusual payroll, or an integration a product will not build for one customer.

How long before a custom build is usable day to day?

Ten to fourteen weeks for a single clinic and longer where payroll and several centres are in scope. Phase it so records and the course balance go live first: those are what the front desk is currently doing on paper, and they make every later screen honest.

Can it run on a phone between patients?

It has to. Home visits and rooms rented by the hour are common, so a system that only works at a desk is a system filled in from memory that evening. Anything clinical needs to be usable one-handed on a phone, offline-tolerant, and quick enough to complete between two patients rather than after both.

Who owns the code at the end of a custom build?
