The market
How physiotherapy clinics in India actually operate.
A physiotherapy practice bills by session, not by visit, and a patient is on a course of 8 to 12 sessions rather than a one-off appointment. Almost every generic clinic system in India models the appointment and not the course, which is why physio front desks end up tracking sessions-remaining in a paper register alongside the software they paid for.

Assessment
First visit is 45 to 60 minutes and produces a case sheet, not a prescription — range-of-motion measurements, pain scale, and a provisional course length.
Course planning
The physiotherapist commits to a number of sessions and a frequency. This is the commercial unit. Most clinics sell it as a package and collect part of it upfront.
Session delivery
Each session logs what was done, what changed, and the home exercise assigned. Sessions are often delivered by a different therapist from the assessor.
Home programme
Exercises are handed over as video or print. Adherence between sessions is the single largest driver of outcome and the thing nobody tracks.
Reassessment and discharge
Repeat the assessment measures, compare against baseline, and discharge with a written summary. Clinics that skip the written discharge lose the referral.

What it has to talk to.
- The system holding the course plan
- A physiotherapy reminder is only useful if it knows which session of the course this is. "Session 4 of 10 tomorrow at 6pm" gets a different response from "you have an appointment", because it reminds the patient they are partway through something they have already paid for. A scheduling tool that only knows about slots cannot say it.
- The exercise library the therapist prescribed from
- The between-visit message is worth sending only if it opens the specific exercise the therapist demonstrated, at the sets and reps they set. A generic "do your exercises" trains patients to ignore the channel — and it is the same channel the reminders use.
- UPI payment links against the session
- Physiotherapy is usually paid per session or per package in cash at the clinic, so reconciliation happens from memory. A link attached to the confirmation turns an outstanding balance into something settled on the patient’s phone before they arrive, timestamped against the session rather than the day.
- Google review requests, fired at discharge
- The moment a patient is most willing to leave a review is the day they are discharged pain-free, and discharge is a state in the course plan rather than a date. Asking after session one, which is what a calendar-driven tool does, collects reviews from people still in pain.
Constraints
What changes the build.
- Physiotherapists are not prescribers
- A physiotherapy record is a case sheet and treatment plan, not a prescription. Software that presents a prescription pad puts the practitioner in a regulatory position they do not hold. The document model has to reflect that.
- Multi-therapist attribution
- Sessions in a single course are delivered by different therapists. Both payroll and clinical accountability need per-session therapist attribution, not per-patient.
- DPDP Act consent for patient data
- Patient records and exercise videos are personal data. Consent capture, purpose limitation and a deletion path need to exist in the data model from the start, not be retrofitted as a privacy page.

A worked example
One engagement, in three parts.
- Context
- A clinic was copying the next day’s appointment list into WhatsApp by hand every evening. The messages said when, never where in the treatment the patient was, and attendance fell away in the middle of every course.
- What was built
- Reminder logic moved onto the course plan rather than the calendar, so a message names the session number, and the prescribed home programme became shareable as video from inside the session screen with a payment link on the same thread.
- What changed
- The evening copying job stopped existing. The clearest signal was that the clinic stopped asking for a reminder feature and started asking for the course view behind it — the message had become useful enough that its source data was the interesting part.
What goes wrong
The mistakes we see in physiotherapy clinics.
- Sending reminders from a marketing tool rather than from the course plan, so the patient gets "you have an appointment tomorrow" with no idea which session it is. If the messaging tool cannot see the plan, fix that before writing any templates.
- Reusing the patient contact list for offers and camps because it is already in the WhatsApp Business account. Consent to run someone’s course does not extend to promoting a knee camp, and it gets templates rejected and the number reported.
- Automating the answer to "is this normal after my session?" because it is the most frequent question in the inbox. Post-session soreness is sometimes expected and sometimes a sign the load was wrong, and that is clinical judgement about one patient.
Questions
What physiotherapy clinics ask.
Can WhatsApp reminders reduce no-shows across a course of physiotherapy?
They move attendance most when the message names where the patient is in their course rather than just the time. A course of eight to twelve sessions has a predictable drop-off around session four, once the initial pain has eased and the rest feels optional, and "session 4 of 10" speaks to exactly that. Reminders will not rescue a course the patient has stopped believing in.
Can we send exercise videos to patients on WhatsApp?
Yes, and it is the most useful thing to send. Send a link into the patient’s own plan rather than the video file, so the therapist can change the programme without resending anything and so the clip is not forwarded on with a patient’s name attached.
Do we need the WhatsApp Business API or is the free app enough?
A solo practitioner sending a handful of messages a day manages on the Business app. Automation needs the API, because only the API sends a template message triggered by something happening in another system. The practical trigger is somebody copying tomorrow’s list into WhatsApp by hand every evening.
What does Meta charge on top of the build?
Meta bills per conversation at its own published rates, separately from anything MyFloww quotes, and a utility conversation such as an appointment reminder is priced differently from a marketing one. For a clinic running a few hundred sessions a month it is a small monthly figure, but it belongs in the business case rather than being discovered afterwards.
Can a patient reply and reach an actual therapist?
That should be the default. The automation handles reminders, confirmations, exercise delivery and payment links; anything clinical is handed to the treating therapist with the case sheet in front of them. A channel nobody answers trains patients to stop reading it, which costs more than the reminders gain.
