How physiotherapy clinics in India actually operate.
A physiotherapy practice bills by session, not by visit, and a patient is on a course of 8 to 12 sessions rather than a one-off appointment. Almost every generic clinic system in India models the appointment and not the course, which is why physio front desks end up tracking sessions-remaining in a paper register alongside the software they paid for.

Assessment
First visit is 45 to 60 minutes and produces a case sheet, not a prescription — range-of-motion measurements, pain scale, and a provisional course length.
Course planning
The physiotherapist commits to a number of sessions and a frequency. This is the commercial unit. Most clinics sell it as a package and collect part of it upfront.
Session delivery
Each session logs what was done, what changed, and the home exercise assigned. Sessions are often delivered by a different therapist from the assessor.
Home programme
Exercises are handed over as video or print. Adherence between sessions is the single largest driver of outcome and the thing nobody tracks.
Reassessment and discharge
Repeat the assessment measures, compare against baseline, and discharge with a written summary. Clinics that skip the written discharge lose the referral.

- Google Business Profile, as the primary ranking asset
- For a physiotherapy clinic the map pack is the search results. Category, service list, photos and review velocity on the profile move more than anything on the website, and the retainer’s first month is usually spent on the profile rather than on content.
- Search Console, segmented by condition
- The reporting question for a physio clinic is not "how is the site doing" but "which conditions are we visible for in which area". Grouping queries by condition is what turns a ranking report into a decision about which page to write next.
- Practo, Justdial and Lybrate profiles
- Directories occupy several of the first ten results for almost every physiotherapy query in India. Consistent practitioner names, qualifications and registration numbers across them corroborate the clinic as one entity instead of three near-duplicates, which is what lets the clinic’s own site take the remaining positions.
- The answer library on the clinic’s own domain
- Condition pages win the transactional query; the library wins the question that comes before it — how many sessions, will it hurt, can I drive after. Those are the pages an AI answer engine quotes, and they carry the clinic’s name into answers where no click happens at all.
Constraints
What changes the build.
- Physiotherapists are not prescribers
- A physiotherapy record is a case sheet and treatment plan, not a prescription. Software that presents a prescription pad puts the practitioner in a regulatory position they do not hold. The document model has to reflect that.
- Multi-therapist attribution
- Sessions in a single course are delivered by different therapists. Both payroll and clinical accountability need per-session therapist attribution, not per-patient.
- DPDP Act consent for patient data
- Patient records and exercise videos are personal data. Consent capture, purpose limitation and a deletion path need to exist in the data model from the start, not be retrofitted as a privacy page.

A worked example
One engagement, in three parts.
- Context
- A clinic ranked for its own name and for nothing a patient in pain would type. It had one Services page, a Google Business Profile with no categories set, and three directory listings spelling the lead physiotherapist’s name three different ways.
- What was built
- The profile was completed and categorised first, the directory listings were made to agree on names and registration numbers, and a page per treated condition was published one at a time with the fee and the usual course length stated on each.
- What changed
- Visibility moved on condition-plus-area queries rather than on the brand term, which is the only movement that brings a new patient. The directory clean-up did more in the first month than any page did — the clinic had been three entities to Google.
What goes wrong
The mistakes we see in physiotherapy clinics.
- Writing a page per neighbourhood the clinic has no premises in. Those are doorway pages by Google’s own definition, they contradict the clinic’s own contact page, and they are the single most common reason a physiotherapy site gets filtered.
- Chasing "physiotherapy near me" as the target query. The map pack absorbs it and a clinic site rarely wins it; the winnable queries are the condition plus the area, and there are dozens of them.
- Leaving the Google Business Profile half-filled while paying for content. For a local clinic the profile outranks the site for most of what matters, so a retainer that starts anywhere else is spending the first month in the wrong place.
- Buying reviews or asking for them by broadcast. Review velocity is a ranking input, which is exactly why a sudden batch of them is a detectable pattern rather than a shortcut.

How does a physiotherapy clinic rank in the Google map pack?
Proximity, relevance and prominence, in that order of stubbornness. Proximity cannot be changed, relevance comes from the profile categories and services matching what people search, and prominence comes from review velocity and consistent citations across directories. Most clinics have never set their categories properly, which is the cheapest thing to fix first.
What are the highest-value keywords for a physiotherapy clinic in India?
Condition plus locality — "frozen shoulder treatment in <area>", "sciatica physiotherapy near <landmark>" — rather than "physiotherapy clinic". They have lower volume individually and there are dozens of them, they convert far better because the searcher has already named their problem, and they are still winnable against directories.
How long before SEO shows results for a clinic?
Four to eight weeks for profile and citation work to move map-pack visibility, and three to six months for condition pages to rank. Anything promising faster on a new domain is describing paid search. Indexation moves months before traffic does, which is the signal worth watching early.
Is GEO different from SEO for a physiotherapy practice?
It is the same content shaped for extraction rather than for a click. An AI answer about a condition cites pages that answer completely in the opening paragraph and use real headings, lists and tables. For a clinic the practical difference is writing the course length, the fee and the recovery expectation as facts a model can lift.
Do we need to blog every week?
No, and it is usually the wrong instruction. A physiotherapy practice has a finite set of conditions and a finite set of questions patients ask about them. Twenty pages that each answer one of those properly outperform a weekly post indefinitely, and they stop needing to be written.
